A 62-year-old woman complained of lower abdominal pain from two months ago. A barium enema revealed narrowing over the entire circumference of the descending colon. The histpathological diagnosis was malignant lymphoma classified as diffuse large cell lymphoma. Immunohitiochemically, tumor cells stained for CD3 and UCHL, but were negative for L26 and MB-1, thus indicating T-cell malignant lymphoma. CHOP therapy was started, but no effects were demonstrated after two courses. After that, carbo-ESHAP therapy was started for two courses. However, the patient developed narrowing of the colon and underwent a left hemicolectmy (D2). Pathological examination of the resected specimens revealed that the tumor was completely replaced by scar tissue with no viable cancer cells. Post-operative chemotherapy was not performed, and the patient has survived without recurrence for 48 months after the operation. Primary T-cell malignant lymphoma of the large intestine is very rare, and its prognosis is much poorer than that of B-cell lymphoma. However, intestinal narrowing after chemotherapy is very rare, but it must be kept in mind as an important complication in the treatment of the disease. |
