A case of enteropathy-type T-cell lymphoma (EATCL) of the jejunum in a patient with HTLV-1 infection is reported. A 60-year-old man who was on radiochemotherapy for stage IIIB pulmonary cancer and had a history of gastric ulcer presented with acute abdominal pain.The abdomen was board like and there was rebound tenderness. The patient complained of intermittent abdominal pain. He had no particular family history. Blood analysis revealed findings of inflammation and HTLV-1 antibody positive. Abdominal CT scan showed free air and thickening of the gastric wall. The most likely diagnosis was gastrointestinal perforation and the patient underwent emergency exploratory laparotomy. The intraoperative findings revealed intestinal multiple submucosal tumors with a perforation at the jejunum about 60 cm apart from the Treiz ligament. A partial resection of the small intestine was performed. Immunohistochemistry revealed EATCL (CD3 + CD4- CD8- TIA-1+) with HTLV-1 infection.Commonly the background of EATCL is celiac sprue, but it is still unknown in the Japanese patients. Herein we report a case of EATCL with HTLV-1 infection, without celiac sprue, with a review of the Japanese literature. |
